Back to school reminder!

Parents, grandparents and other responisble parties. Remember, along with all of your other Back to School paperwork, to get a copy of our Authorization to Release Information form.

This must be filed with your child's school as early as possible. This form is required for us to speak with your child's teacher about grades, assignments and progress both in school and at Sylvan, so it is very important!

If your child is on an IEP or 504 plan or in enrolled in a Study Skills or Homework Support program, this form is critical to our success.

For all enrolled students who have a signed authorization on file, regardless of program, we maintain weekly or bi-weekly contact via email, fax or phone with classroom teachers and also are available to attend any and all parent teacher conferences, including IEP and 504 meetings.

This service is provided at no additional charge. You can ask for a copy of the form from Lauren Covington or Elizabeth Lewis at Sylvan. Once you fill it out, turn it back in to us and we'll go ahead and file it with the school.
